To further protect all residents in Ontario against COVID-19, the government will require people to be fully vaccinated and provide proof of their vaccination status to access certain public places starting September 22, 2021. By providing proof of vaccination, residents in Ontario are helping to reduce the risk of spreading COVID-19, and encourages non-vaccinated individuals to get their vaccination shots. 

Proof of vaccination is not only important for Ontario residents, but it may also be required to maintain/obtain work, attend school, and/or travel outside the province, state, or country.

NOTARIZING YOUR PROOF OF VACCINE AND/OR COVID-19 TEST RESULTS

At Notable Notary we can help you meet these requirements by providing you with a certified true copy of the below documents:

  • Vaccine Receipt – Your proof of vaccination receipt; details of your vaccination (dose info, date/time of vaccination, location, etc.)
  • Vaccine Status – Your vaccine passport, document, status card, receipt, etc.
  • COVID-19 test results – Your positive or negative COVD-19 test results

A certified true copy is an exact copy of a document that has been certified by a notary public to help deter fraud. These certified copies can be used for domestic and international use, and are just as good as using the original document itself.

ALTERNATIVES TO PROOF OF VACCINE AND/OR COVID-19 TEST RESULTS

If you do not have any of the above mentioned documents, but are still required to provide proof of vaccination or COVID-19 test results, you may be asked to provide an affidavit regarding your vaccine status. 

Many employers, universities, colleges, and other institutions may request this vaccination attestation as an alternative to proof of vaccination issued by the provincial government. Most of these institutions will provide you with the affidavit they would like to be completed but if not, Notable Notary provides free online templates that can be used instead. 

Affidavits are commonly used to allow a person to declare something to be true for the purposes of satisfying a legal requirement or regulation when no other evidence is available. These statements must be declared in front of a notary public or commissioner of oaths.
